You can have it printed so you can fill out the said form. In Part V for this form entitled "EMPLOYER INFORMATION". This portion should be filled up by your EMPLOYER not you. This is required for those with EMPLOYERs.
The required documentary attachment for this application is your MARRIAGE CERTIFICATE.
If a Taxpayer has no EMPLOYER or has a business. The BIR Form 1905 should be used to UPDATE his or her status. In Part II of this form check letter "K" and in the provide blank box write "UPDATE STATUS & REGISTERED NAME TO MARRIED NAME". The second page or back side of this form in "4G" check REGISTERED NAME. And in the provided boxes in "NEW" write your MARRIED NAME and in OLD your MAIDEN NAME. Attached marriage certificate.
TAKE NOTE: You should submit this application form to the BIR branch office or Revenue District Office where your TIN is currently registered.
